I hereby state that I am in a good health condition and I am not suffering from any kind of mental, physical or genetical disorders, anorexia, bulimia; I am not pregnant and am not using any prescribed medication or drugs at the present time.
I am taking full responsibility for my participation in the workshop.
From the beginning to the end of the workshop it is strictly forbidden to use alcohol, drugs (in any form), cigarettes, to have sex or to read. In addition it is also forbidden during the advised period to talk, or to consume any kind of food except large amounts of water and herbal teas.
I hereby state that I agree to give permission to publish any material I produce during the workshop to Marina Abramovic.
